2025 Membership $10 payment with Interac or Cheque

Use this form only if you are paying the $10 new membership or renewal fee by Cheque or Interac e-Transfer. To avoid confusion in processing your payment, this form should be completed and submitted before mailing a cheque or making an e-transfer.
